Lady of Fatima Design Competition Entry Form

1. To submit Artwork through e-mail please take a photo or a scan of your Artwork and e-mail the images to the email address  artspacelima1990@gmail.com and follow instructions below. Call 419-222-1721 for any questions.

2. Subject line must read: “2024 Shrine Art Contest Submission: (Artist’s First and Last Name)”.

3. Accepted file types: .jpg, .pdf, .png. File size must be less than 10MB - each document.

4. Must attach a completed Entry Form as part of the e-mail. To download click here 

5. If chosen as a winner in an Age Category, the Artist’s original artwork must be delivered (by mail or hand delivery) to the address at the end of this form for display at ArtSpace/Lima. Inability to submit artwork will result in disqualification. [NOTE: do not send original artwork unless – and until – you are notified that your Design Submission is a winner].

6. By submitting a Submission of Artwork, the Artist attests that: a) the Artwork is Artist’s original work; b) the Artwork is does not infringe on any copyrights or patents; c) the Artwork is may be displayed by ASE and ArtSpace/Lima; d) the Artist’s name and town of Ohio residence, and his or her likeness may be used by the Sponsors on the Sponsor’s respective websites, in connection with press releases, when displaying the Artwork, and other related promotional items; and, e) if chosen as a winner and Artist accepts the prize (stipend/honorarium), ASE owns the Artwork and all ideas and design concepts represented therein, with the right to use any, all, or none of the ideas contained in the Artwork.

7. Entry Deadline is 5:00 p.m. EST on Thurs. Aug. 15, 2024 (the “Due Date and Time”).

8. The signatures of the Artist and any parent, guardian or legal representative may be done by electronic means (or “wet signature” and scanned).

All communications from the Sponsors regarding the contest, including announcement of winners, will be sent to the Artist (unless under age 18, when sent to the minor Artists’ parent, guardian, or legal representative) using the email address(es) designated on the Entry Form.

A Design Submission by the Artist does not assure the Artist that his or her entry will be used in whole or in part for the Final Design of the Shrine. The Sponsors reserve the right to reject any and all Design Submissions.

The Sponsors are both non-profit organizations. No member of the Jury is compensated for his or her participation in this Design Competition.

In Zivich v. Mentor Soccer Club, Inc., 82 Ohio St. 3d 367, 372, 696 N.E.2d 201, 205 (Ohio 1988), the Ohio Supreme Court has held that a consent or release signed on behalf of a minor child by a parent, guardian or legal representative in favor of a non-profit organization is valid. Because of the agreements in this Entry Form, the Sponsors are able to offer the opportunity for a minor child to participate in this state-wide Design Competition without the risks and overwhelming costs of litigation to the Sponsors. Public policy does not forbid such an agreement. In fact, public policy supports it. The enforcement of these agreements may well promote more active involvement by participants and their families, which, in turn, promotes the overall quality and safety of these activities.

For all Entries regardless of Age Category:

The person(s) signing above hereby consent to the use of the Artist’s name and town of Ohio residence by the Sponsors on the Sponsor’s respective websites, in connection with press releases, when displaying the Artwork, and other related promotional items.

The above consents require no further permission, compensation or advance approval by a Sponsor or the media.

By submitting this Entry Form, the Artist (and his or her parent, guardian and/or legal representative if Artist is under age 18), including each member of a Design Team, warrant that:

1. the Design Submission is the Artist’s original work;

2. the Artist has been a resident of the state of Ohio for at least six months in the year immediately preceding this Design Submission;

3. the Artist is (or will be) the age stated above (as of Aug. 15, 2024) whose birthdate is accurately listed above;

4. the person(s) signing this Entry Form are authorized to sign this form in such capacity;

5. the Artist has read the Design Competition Rules and agrees to those terms;

6. if the Artist is chosen as a winner, Artist assigns all copyright, patents, trademarks and other intellectual property rights related to the Design Submission to ASE and;

7. upon acceptance of the prize (stipend/honorarium) the Artwork submitted by the Artist becomes the property of ASE.

[Note: if the Design Submission is for a Design Team, then each member of the Design Team must be qualified by Age Category and Ohio residency]

In consideration for the participation in this contest, Artists and (if under age 18) the named parent, guardian, and/or legal representative release, discharge, indemnify and agree to hold harmless the Sponsors, their employees, officers, directors, trustees, agents, and partners from all claims, damages, demands, obligations, or losses of any kind related to this Design Competition. The Sponsors may rely upon the representations by the Artist (and, if applicable, by the Parent, Guardian or Legal Rep.
